Student Chosen Name Policy

Policy Statement


Life University recognizes that students may wish to use a name other than their legal name on various University documents and platforms. When designated by a student, the University will use a chosen first/middle name except where use of the legal name is required by University business or required by law.  It is the expectation that members of the Life University community respect the use of the student’s chosen first/middle name. The chosen name will be used as outlined in this policy.

 

Procedure(s)


Chosen Name Guidelines

Students may add a chosen first and/or middle name through completion of a DocuSign Form regardless of whether or not they have legally changed their name. The Registrar’s Office will accept the Chosen Name Form up until the last day of each quarter, to be effective for the following quarter. This name will appear alongside a student’s legal name of record, which will remain on file.   Chosen  names  that  differ  from  an  individual’s  legal  name  will  only be  used  on Life

University’s campus in situations where the student’s legal name is not required by University business or required by law. Specifically, a chosen name will be viewable in parts of Colleague, EagleNet, and Blackboard.  All external reporting of student name must, by law, use legal name.

Places Where Legal First/Middle Name Must Be Used

  • Student Financial Accounts
  • Financial Aid
  • Responses to enrollment enquiries such as verification requests
  • Official transcripts
  • Student employee/payroll information
  • Emergency Responder and Enforcement Systems (Parking, Police, Campus Safety etc.)
  • Internal systems that require and can only accommodate Legal Name
  • All external communications and reporting
  • Mail sent to the home address on record
  • Designated student email address
  • Designated student user name

Faculty, staff, and parents who have access to the student’s school record will see the chosen name in parts of Colleague, EagleNet and Blackboard.

Life University reserves the right to deny the use of or remove any chosen name from University record for misuse, as determined by the Dean of Students.
